Clinical Results and Review of Techniques of Thoracic Endoscopic Unilateral Laminotomy with Bilateral Decompression (TE-ULBD) Using the Outside-In Technique for Thoracic Ossified Ligamentum Flavum

Objective: Uniportal full endoscopic thoracic surgery can be performed through transforaminal and interlaminar approaches. The approach is commonly described as unilateral laminotomy for bilateral decompression (TE-ULBD), which typically indicated pathologies such ossified ligamentum flavum other posteriorly based compressive pathologies. TE-ULBD decompresses the central lateral recesses of spinal canal. Both outside-in (over top flavum) inside-out (under approaches decompress canal uniportal route.Methods: A retrospective clinical cohort evaluation patients who underwent was from January 2018 to December 2021 Results: 50 cases with a mean age 65 years old were evaluated. complication rate 5.4% reoperation 2%. Statistically significant VAS improvements found at 1 week, 6 months, final follow-up, changes 3.95 ± 1.49, 4.95 1.7, 5.2 1.8 points, respectively. Likewise, Oswestry Disability Index 3 follow-up 33.8 9.05, 40.12 10.38, 41.92 11.26, respectively (p < 0.001). Significant in cross-sectional area upper endplate, mid-disc, lower endplate (57.62 50.6, 89.86 55.93, 64.93 60.91 mm<sup>2</sup>, respectively; p Conclusion: using technique could achieve good outcomes low complications our patients.
